Integrated Behavioral Health Providers work closely with the health center's interdisciplinary team to assess and provide behavioral health treatment in the primary care setting.
Under the direction of the Chief Medical Officer, this position provides comprehensive, coordinated primary and preventive healthcare services to an ethnically diverse patient population within their appropriate scope of practice. This position supervises the behavioral health team including Psychologists, and Addiction Counselors
In this role your responsibilities include:
Evaluates and treats patients within the licensed scope of practice; appropriately refers patients for specialty evaluation and treatment; provides preventive health advice and case management; and provides consultation to other staff.

Participates in multi-disciplinary, team-based care of patients.
Participates in training, reporting, and other requirements of various grant-supported programs.
Participates in on-site and off-site outreach and continuing education programs and ensures training to maintain license and meet the population's needs.
Responsible for accurate, timely, and complete documentation in patient records in accordance with Uptown Community Health Center policy.
Renders care to UCHC patients in accordance with quality and standards as established by UCHC, the standards of care prevailing in the community, applicable law, and according to state and federal grant requirements for non-discrimination regardless of status, race, sex, religion, national origin, handicap, age, or ability to pay.
During each patient visit, provide patients and family members with counseling and education regarding diagnosis, treatment, and appropriate follow-up. Verify and document that the patient understands the diagnosis, treatment, and follow-up recommendations.
Serves as a role model for effective staff teamwork and actively maintains positive morale and attitude.
Qualifications:
Licensure/Certifications:
Valid license to practice the profession in the Colorado.
Board certification/eligibility based on education and specialty.
Valid DEA in Colorado.
Required:
Medical Degree (MD or DO) from a fully accredited medical or osteopathic school of medicine.
Residency training in the area of specialty.
Preferred:
Experience with Epic application and health record.
Experience working with and sensitivity toward special populations, including those with limited income, limited education, and non-English speaking individuals.
Bilingual English / Spanish, complete comprehension, literacy, and fluency in both languages.
