Job Summary

We are looking for individual to join our client’s team as Pharmacist. You will be preparing and dispense prescriptions, ensure medicines and doses are correct, prevent harmful drug interactions, and counsel patients on the safe and appropriate use of their medications.

 

Job Description

A. Unit Dose Responsibilities:

Pharmacist prepares and dispenses medication orders per physician request according to established policies, procedure, and protocols.

  • Review and accepts orders on the hospital computerized physician order entry (CPOE) for accuracy prior to dispensing.
  • Fill unit-dose medication carts by each nursing unit.
  • Compound and dispenses pharmaceuticals, including sterile, chemotherapy, and parenteral nutrition preparations accurately.
  • Issues controlled substances to patient care areas and maintains records as required by law.
  • Communicate pending issues to other pharmacy personnel prior to leaving each shift (hand-off).
  • Replace medications removed from emergency carts accurately within established time frames.
  • Ensure integrity of emergency cart locks.
  • Replace expired medications in emergency carts.
  • Ensure that emergency cart expiration date is within time period specified by departmental policy.
  • Prepare requests for medication from the stock room.
  • Store medications and hazardous chemicals safely and securely
  • Review STAT orders within I-hour period.
  • Review non-Stat order within 2-hour period.

B. Non-Unit Dose Responsibilities:

Ensure safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols. Know the policies and procedures of the department and hospital standards.

  • Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, allergies, appropriateness of drug and dose, and therapeutic duplications.
  • Contact prescribers when needed and document interventions.
  • Detect and reports suspected adverse drug reactions and medication errors in a timely manner.
  • Sustain the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.
  • Provide clinical consultation and clarification to practitioners. Suggests appropriate cost-effective therapeutic.
  • alternatives to medical staff as needed.
  • Provide drug information to medical staff, nursing, other healthcare professionals, patients, and their families.
  • Document all clinical activities and interventions accurately and completely.
  • Participate in the quality improvement and medication use review activities of the department. Collects data; conducts quality monitors and inspections; and maintain logs, records, and other documentation as assigned.
  • Attend pharmacy staff meetings.
  • Complete all competence/skills assessment requirements.
  • Perform other duties as assigned by supervisor.
  • Abide to National Patient Safety Goals (NPSGs),
  • Convert IV to PO regimen for patient safety.
  • Arrive to work area on a timely basis.

 

Skills Required

  • 1 year Pharmacist experience
  • Pharmacy dispensing system experience, Sterile Processing experience, IV room experience, knowledge of an EMR (Electronic Medical Records system) preferred.
  • Must have hospital experience as Pharmacist.

 

Education/Training/Certifications

  • Valid license to practice as a pharmacist issued by NYSED, Primary Source Verification required.

 

Additional Requirements

  • Malpractice Insurance - All Credentialed Providers are required to have active individual Medical Malpractice.
  • Schedule Notes: 3:30 PM-12:00 AM

 

  Pay Rate

  • $81.00 per hour
