Job Title: Person Centered Plan Coordinator 

Employer: Chesterwye Center, Inc.

Reports to: Director of Quality Enhancement 

FLSA Status: Exempt 
Hours: Full-Time

Time: 8:00 am to 4:00 pm or 9:00 am to 5:00 pm; flex time negotiable after completion of probationary period 

Salary: 48,000 to $52,000 

Effective Date: 06/17/2025

Job Summary: Chesterwye Center is a non-profit organization working to serve individuals with intellectual disabilities to support them in achieving the goal of leading healthy and fulfilling lives, which is integrated within their community. The Person Centered Plan Coordinator is responsible for ensuring that all the support needs of a person receiving services are met by developing the agency's plan to meet the person's choices. This is documented in the person’s person-centered plan (PCP). The Person Centered Plan Coordinator functions as an expert in person-centered thinking. They are responsible for all reporting, goal submission, monitoring documentation, including ongoing program data, as well as the monitoring of established goals and the implementation of various plans. They serve as Chesterwye clinical facilitator by providing, coordinating, monitoring, advocating, negotiating, training, and referring to support services, as well as assessing the needs of the individuals we serve. 

Job Responsibilities and Duties: 

  • Manage the Person-Centered planning process for approximately 60 individuals supported who receive Residential, Meaningful Day, Personal Supports, Employment Services, Self-Direction, Respite Services, etc. 
  • Will train all staff working directly or indirectly with individuals on all components and details of the PCP and BSP (Behavioral Support Plan) of the individuals. Emphasizing the Chesterwye expectations regarding the individuals served as they relate to the PCP, BSP, and Person-Centered thinking. 
  • Collect and coordinate the PCP preparation, SIP review, Behavior Plans, Nursing Plan of Care, Assessments etc. 
  • Consults with direct manager and Program Managers during all phases of the PCP process, including Pre-PCP meetings, the assessment with goal planning phase, and the implementation phase. 
  • Initiate modification of supports and or funding requests with the Coordinator of Community Services (CCS) case managers, with input from Program Managers and oversight from the direct supervisor. 
  • Maintain ongoing positive communication with DDA, community coordination agencies, and other service providers on behalf of people being served. 
  • Maintain up-to-date records and documents in iCare manager, program files, including the PCP and BSP documentation, entry of PCP and BSP, face sheet, tracking charts, and other documents relevant to each person's services. 
  • Works closely with Quality Enhancement in evaluating programs and establish new systems. 
  • Driving is required for this position to include personal or agency vehicles. Must have a minimum of two years of driving experience, a valid driver's license issued by the state of residence, and no driving or licensure restrictions that would impede the driver's ability to operate the vehicle as required by the agency. The driving record must not have two or more points to be eligible for this position. 
  • Monitor and assist in the submission of documentation for eligibility paperwork (i.e. SSI, SSA, Medicaid waiver conversion, medical assistance, food stamps, HUD). 
  • Performs all other duties as assigned, as necessary to fulfill the mission of Chesterwye Center. 

 

Requirements

  • Bachelor’s degree in Human Services, Communication or High School Diploma with equivalent experience in the field 
  • Minimum three (3) years working with the IDD population desired 
  • Must have a valid driver’s license and daily access to reliable personal transportation, and drive the people we support as necessary, to fulfill the duties and responsibilities of the position. Requires regular travel throughout the assigned geographical area. 
  • Become thoroughly trained in person-centered thinking. 
  • Interest in or passion of supporting people with intellectual and developmental disabilities in achieving their dreams/goals required.
