IMCS GROUP is seeking a local contract nurse RN Home Health for a local contract nursing job in Napa, California.

• Administering medication to patients.
• Creating reports to track the patients' progress.
• Working with other professionals to create treatment plans.
• Cleaning and re-dressing wounds if needed.
• Measuring vital signs.
• Monitoring changes in behavior and progress.
• Perform comprehensive patient assessments, including physical, psychosocial, and environmental evaluations.
• Develop and implement individualized Plans of Care (POC) in collaboration with the physician and interdisciplinary team.
• Provide skilled nursing services, such as wound care, IV therapy, medication management, catheter care, and chronic disease monitoring.
• Educate patients and caregivers on disease management, medication regimens, and home safety to promote independence.
• Accurately document all clinical visits and communications in HCHB EMR systems in a timely manner.
• Communicate changes in patient condition promptly to physicians and obtain/implement new orders as necessary.
• Collaborate with therapists, social workers, home health aides, and other care providers to ensure coordinated care.
• Maintain compliance with Medicare, state, and agency regulations, infection control policies, and safety standards.
• Participate in case conferences, quality assurance programs (QAPI), and continuing education activities.

Must have CA State RN license\nMust have BLS AHA\nMust have Home Health experience with HCHB EMR.
