We are seeking a highly skilled and motivated Specialty Pharmacist to join our team at Christ Community Health Services, Inc (CCHS). As our Specialty Pharmacist, you will play a crucial role in developing the program and providing comprehensive pharmaceutical care to OUR patients with complex medical conditions, particularly those requiring specialty medications. You will collaborate closely with healthcare providers, patients, and community resources to optimize medication therapy and improve patient outcomes. This role will be expected to lead the development and implementation of new and current programs to obtain and maintain accreditation by accrediting bodies, such as Utilization Review Accreditation Commission (URAC) and Accreditation Commission for Health Care (ACHC).

KEY RESPONSIBILITIES

1. Medication Therapy Management: Conduct comprehensive medication reviews for patients receiving specialty medications, ensuring appropriateness, efficacy, and safety of therapy.

2. Patient Education and Counseling: Provide thorough education and counseling to patients and their caregivers regarding their specialty medications, including administration, potential side effects, adherence strategies, and proper storage.

3. Care Coordination: Collaborate with healthcare providers, including physicians, nurses, and other allied health professionals, to develop and implement individualized care plans for patients receiving specialty medications.

4. Prior Authorization Assistance: Assist patients and providers with navigating the prior authorization process for specialty medications, including completion of necessary paperwork and communication with payers.

5. Medication Access Support: Work closely with pharmaceutical manufacturers, specialty pharmacies, and insurance companies to ensure timely access to specialty medications for patients, including assistance with copay assistance programs and alternative funding options.

6. Adherence Monitoring: Utilize various tools and resources to monitor patient adherence to specialty medication therapy and implement interventions as needed to improve adherence and outcomes.

7. Quality Improvement Initiatives: Participate in quality improvement initiatives aimed at optimizing medication use, enhancing patient outcomes, and reducing healthcare costs within the FQHC setting.

8. Continuing Education: Stay abreast of current trends, guidelines, and developments in specialty pharmacy practice through continuing education, literature review, and participation in professional organizations. May be required to attend conferences to keep up with changing requirements.

9. Complies with federal and state requirements and regulations.

10. Performs other duties, as required.

  • Degree from an accredited college of pharmacy
  • Doctor of Pharmacy, preferred
  • Certification in Specialty Pharmacy, preferred
  • Experience (2 years) managing patients’ specialty medication needs
  • Ambulatory or retail practice experience (2 years) preferably
  • Excellent verbal and written skills
  • Proficiency with Microsoft Office, Therigy
