What You Will Do
As a School Liaison/Career Counselor, you will work in conjunction with the multidisciplinary comprehensive care team, providing educational support for patients and families affected by non-malignant hematologic disorders.
The Opportunity
Participate in clinic to identify patients in need of education services. Collaborate with the multidisciplinary team to establish an plan appropriate to patient needs.
Provide Daycare and Preschool orientations as well as collaborate with primary, secondary and post-secondary schools to ensure the educational plan accounts for a patient’s hemoglobinopathy and special needs and conforms to ADA and/or APA rules and procedures.
Provide guidance for patients interested in post-secondary education or vocational training by providing resources on planning, financial assistance, scholarship opportunities, resume writing and interviewing skills.
Serve as a patient advocate to support patients, families , school teachers and administrators in incorporating test results into the patient’s educational plan.
Encourage patients to establish career goals by providing emotional support, conducting and analyzing personality and career inventories, identifying potential resources, and assisting patients in completing applications.
Participate in IHTC youth initiatives, including camp and transition activities, and other initiatives as requested
