Avera is currently seeking Registered Nurses (RN) Case Manager – Home Infusion looking to deliver the best possible nursing care in Avera’s patient-focused settings.

Job Summary

The home infusion nurse coordinator serves as a liaison with referral sources, clinical pharmacists, home nursing agencies, and payors to assess candidates for home infusion, initiate therapy, and monitor ongoing care. Collaborates with interdisciplinary care team to encourage the best possible patient outcome, assist with transitions between levels of care, and ensure continuity of care.

Education and/or Experience

• Graduate from a CCNE or NLN accredited nursing program is strongly preferred. Graduate from an approved nursing program is acceptable.


• Bachelor's degree in nursing preferred

Licensure/Certification/Registration

• Current and unrestricted license as a registered nurse (RN) in the State of South Dakota and/or meeting the requirements of multi-state licensure is required.

Work Schedule

• Monday – Friday


• 80 hours per 2 weeks

Pay

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

• $29.50 - $44.25 per hour
