Registered Nurse (Home Health Case Manager) - Fort Wayne, IN
Shifts: Days (Mon-Fri) 9:00 AM to 5:00 PM
Job Description:
We are looking for a Registered Nurse (Home Health Case Manager) to provide medication box fills, assessments, computer charting. Who is also in charge of coordination of care with physicians.
Roles and Responsibilities:
• Coordinates total patient care by conducting comprehensive health and psychosocial evaluation, monitoring the patient's condition, promoting sound preventive practices, coordinating services, and conducting teaching and training activities.
• Evaluates the effectiveness of nursing services to the patient and family on an ongoing basis.
• Performs admission, transfer, re-certification, resumption of care, and discharge paperwork for the home care patient.
• Prepares and presents the patient's record to the Clinical Record Review Committee as indicated.
• Consults with the attending physician concerning alterations of Patient Care Plans, checks with the appropriate supervisor, and makes changes as appropriate.
Qualification/Experience:
• Associate or Bachelor's degree in Nursing
• One or more years of experience in a community/home health agency or in a hospital setting is preferred.
Licenses/Certifications:
• Must be licensed in the state of Indiana as a Registered Nurse
• Must have a driver's license and be willing and able to drive to patients' residences
• Must have a current CPR certification
