We are seeking a total of four Provider Service Liaisons (PSL), the ideal candidates will live in one of the following preferred county areas, as they are central to each geographic territory:
- PSL 1- Gaston or Mecklenburg counties
- PSL 2- Guilford, Randolph, or Moore Counties
- PSL 3- Wake or Johnson Counties
- PSL 4- Pitt, Wayne, or Duplin Counties
Position Summary
The Provider Services Liaison (PSL) serves as an in-person connection for CCPN practices, facilitating strong relationships with the organization and deepening practice understanding and adoption of our materials, resources, and data platforms, including but not limited to Practice Perfect. This position works collaboratively with Community Care of North Carolina (CCNC) statewide teams, and Community Care Physician Network (CCPN) in assigned regions to provide the necessary tools, communication, and resources to enhance engagement and ensure CCPN practice success in value-based care. This position is a “road warrior” with most of the day/week spent visiting CCPN practices in the field, based off the strategic plan set by Population Health Leadership and the Executive Team.
Essential Functions
Serves as point person for practices through the following functions:
- Proactively develops and maintains strong provider relations to increase participation and engagement with CCPN programs through in-person visits.
- In person delivery of information, tools, and resources to promote quality and cost-effective outcomes in alignment with the organization’s value-based care contracts, and in support of the PRR and QIS efforts.
- Supports growth of the network and actively conducts recruitment outreach to enroll eligible practices in CCPN and CCNC contracts and programs in collaboration with the Contracts & Programs team.
- Onboards, educates, and supports practices as necessary, with accessing CCNC’s data platforms, including but not limited to Practice Perfect and Virtual Health. Reinforces regular use of Practice Perfect (monthly at a minimum) and teaches practice staff how to locate resources in the CCPN Private Portal. Assists with ticketing when access needs to be restored, problems arise, or new users need to be added.
- Responsible for conducting CCPN education, training, and on-boarding sessions for new and existing CCPN member practices. These sessions may be warranted when there is turn-over in key practice positions, when a practice wishes to train or refresh their team members on the role and value of CCPN, or when a practice is new to CCPN.
- Shares contracts, flyers, promotional resources and tools with practices to support and reinforce messaging taking place from other CCNC and CCPN teams.
- Works collaboratively with and refers to Subject Matter Experts (SMEs) and other team members as needed to connect the practice to needed support (e.g., Dyad team, QI Specialists, Contract/Program Specialists, EMR Specialists, Billing and Coding Specialists, etc.).
- Upon request, conduct an in-person visit to secure outstanding documents the practice has not submitted (e.g., CCPN Contract, etc.).
- Able to provide hands on assistance with Semi-Annual User Attestation for Practice Perfect Annual Compliance Attestation.
- Delivers provider and practice education, including information on platforms and services, CCNC/CCPN.
- Promotes and attends monthly CONNECTED meetings, and other educational offerings, as appropriate, to ensure understanding of information for dissemination to PCPs.
- Supports recruitment of practice staff members to speak at future events, such as monthly CC Connected meetings, quarterly educational offerings, clinical forums, etc.
- Network with providers and other important contacts for statewide operations, CCNC, and CCPN
- Documents practice interactions in a timely manner according to documentation guidelines.
- Maintains member and family confidentiality in accordance with HIPAA.
- Always represents the organization in a professional manner.
- Extensive and regular travel using personal vehicle will be required each week, with overnight stays at times, and other job duties as required.
Qualifications
- Bachelor’s Degree in a health care field is highly preferred; Associate’s degree with two years of equivalent work experience; OR a minimum of five years of equivalent work experience.
- Experience in sales, healthcare sales, physician practice (medical practice) management, or account management experience highly preferred.
- Experience in working with medical practices is highly preferred.
- Experience documenting in CRM or other tracking software is preferred.
- Experience in marketing and communication techniques such as Motivational Interviewing (MI) and Academic Detailing.
- Experience in data analysis & interpretation highly preferred.
- Experience in quality reporting/EHR technical assistance is highly preferred.
- Maintain a valid driver’s license with current auto liability insurance.

- Computer skills, including fluency in Microsoft Office applications (Word, Excel, PowerPoint, etc.).
- Familiarity with data analysis and interpretation; ability to appropriately communicate results verbally and in writing based on target audience as determined by specific functions of the role.
- Utilization of QI methodologies such as the Model for Improvement with rapid cycle PDSAs and other evidence-based strategies and techniques.
- Comfort with public speaking in front of people across multiple disciplines.
Working Conditions
- The job environment is home based and travel daily for in-person visits with practices is required, practice visits may also be virtual as necessary, however the primary function of the role is building relationships and providing support in person at CCPN member practices.
- Connection to High-Speed Internet required.
- Exposure to general office conditions, as well as communicable diseases could occur.
- Exposure to general office and household conditions including minor physical inconveniences, discomfort in the work setting, or sitting for moderate periods of time.
- Daily travel within the region and/or the state with some overnight stays.
- There is occasional lifting of 20-30 pounds necessary to complete a task.
- Evening and weekend work may be required with advance notice and corresponding adjustment in work week schedule.
