Job Summary

Acute Hospital Care at Home (AHCaH) Advanced Practitioners (APs) operate similarly to inpatient hospitalists with a more virtual care approach. Cooperating with other team members through huddles and as needed to assess patient needs and identify appropriate patients for this model of care at our off-campus office in Danville, PA.

Program Highlights:
The AHCaH program utilizes remote patient monitoring, advanced technology, and interdisciplinary collaboration to deliver exceptional care to patients who meet hospital admission criteria.

Target Population & Conditions Treated:

• Med/Surgical level of care
• Congestive Heart Failure (CHF) exacerbation
• COPD exacerbation
• Urinary Tract Infections (UTIs)
• Other appropriate conditions

Your Role as an Advanced Practitioner (AP):
As the core provider on the AHCaH team, you’ll work closely with patients and collaborate with a dynamic interdisciplinary care team under physician guidance. Key responsibilities include:

• Conducting in-person and remote patient assessments
• Developing and implementing treatment plans
• Providing ongoing monitoring and coordination with field staff

Team Composition:

• Advanced Practitioner (AP): Primary provider managing patient care
• RN Coordinator: Care coordination and program oversight
• Field Staff: Mobile paramedics and RNs for in-home care delivery
• Physician: Direct care and interdisciplinary team supervision

Location:
Based at the Geisinger Health Plan Hughes Center in Danville, PA, with limited travel to Geisinger Medical Center, Geisinger Bloomsburg Hospital, and Geisinger Shamokin Community Hospital as needed.

Schedule:

• Work-life balance: Enjoy a 7 days on / 7 days off schedule
• Hours: 7:00 AM – 7:00 PM
• Call Rotation: Shared with physicians and other Advanced Practitioners

Qualifications:

• Nurse Practitioner or Physician Assistant with relevant certification/licensure
• Clinical experience in acute care or hospital-based medicine preferred
