Summary:
In accordance with the mission and values of The Village of St. Edward, the MDS Nurse Coordinator II (RN) is responsible for preparation and currency of the individualized resident care plans; for preparation and timely submission of the accurate MDS, quality measures and other relevant information; participation and record keeping of interdisciplinary team (IDT) meetings; supervising and directing care provided by nursing assistants; participating in continued professional development; performing other job related duties as assigned; works independently to provide restorative care to residents including assessment, planning, implementation and evaluation.
Qualifications:
Current Registered Nurse (RN) in the State of Ohio
At least two year’s experience in long-term care facility
At least one year of experience in MDS
Obtains Infection Preventionist Certification within 6 months of hire
Good physical and mental health.
Possesses the necessary dexterity to perform injections, IVS, etc.
Able to lift, push and pull 35-50 pounds independently, and over 50 pounds with assistance.
Possesses the ability to bend, stoop, and reach at different angles.
Walks, stands, and sits intermittently during the day.
Must interact effectively with residents, families and public on personal basis.
Demonstrates effective communication skills.
Ability to manage and supervise nursing staff.
Demonstrates evidence of leadership abilities, clinical competence and interpersonal skills.
Knowledgeable of principles of growth and development for the entire life span with the ability to provide care appropriate for the adult and geriatric residents.

Some essential duties of the position, not an all-inclusive list:
Assessment
Assesses the resident to identify their bio-psychosocial and spiritual needs and uses information to complete nursing care plan and MDS.
Interprets and uses diagnostic data specific to residents and correlates with changes in residents' condition.
Assesses the resident to determine unidentified service needs.
Planning/Implementation
Independently or in collaboration with physicians and other members of the interdisciplinary team, develops strategies to address identified problems toward achievement of desired outcomes
Attends and coordinates IDT and family meetings regularly and as requested.
Coordinates actions necessary to achieve desired restorative outcomes.
Develops, maintains and educates Transfer/Restorative programs.
Evaluation
Informs appropriate members of the interdisciplinary team of changes in resident status, needs and effectiveness of interventions and restorative program.
Contributes to episodic care plans of residents.
Resident/family education
Performs consistent ongoing assessment/evaluation of learning needs of residents/families.
Evaluates residents/families understanding of learning activities through their verbalization/demonstration of the concept taught.
MDS/Care Planning
Reviews available clinical data prior to admission and throughout skilled period to determine PDPM Classification for skilled residents; communicates to fiscal in a timely way.
Remains current with changes in MDS manual/guidelines and CMS regulatory changes.
Performs MDS and other assessments per policy/schedule; delegates appropriately.
Coordinates completion of the MDS by the IDT; certifies completion within designated time frames by signature and date.
Coordinates frequent audits of MDS with contracted therapy provider
Submits MDS and other essential documentation per regulations and/or policy.
Initiates and keeps current individualized care plans for each assigned resident.
Coordinates care plan with the MDS for each resident.
