Key Responsibilities:
The RN MDS Coordinator plays a vital role in ensuring accurate resident assessments and the effective optimization of care plans. This position will:
Lead and coordinate the development and completion of resident assessments (MDS) in line with current regulations and guidelines.
Ensure the MDS process is completed accurately and within required timeframes.
Collaborate with the interdisciplinary team to develop personalized care plans for residents.
Continuously monitor and document residents' progress and responses to care.
Actively engage in the Quality Assurance and Performance Improvement (QAPI) process to enhance the overall quality of care.
Maintain clear and effective communication with residents, families, and healthcare providers to ensure seamless care coordination.
Stay up-to-date on MDS regulations and guidelines to ensure compliance with industry standards.
This role is critical in maintaining high standards of care and regulatory compliance within the facility.

Qualifications:
Current RN (Registered Nurse) license required
RAC-CT (Resident Assessment Coordinator-Certified) certification required
Strong understanding of MDS 3.0, RAI process, and care planning
Excellent organizational, communication, and interpersonal skills
Ability to work independently and collaboratively with a team
Commitment to delivering high-quality care to residents
