Job Description

Description of the role: University Home Care is seeking a Licensed Master Social Worker (LMSW) for our location in Livonia, MI. The LMSW will be responsible for providing comprehensive social work services to patients in their homes. Responsibilities:

Conduct initial patient assessments and develop personalized care plans
Provide counseling and support to patients and their families
Coordinate with healthcare providers to ensure holistic care for patients
Advocate on behalf of patients to access necessary resources
Maintain accurate and up-to-date patient records

Requirements:

Valid LMSW license in the state of Michigan
Prior experience in home care or healthcare setting preferred
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
