A difficult task, but it can be accomplished together. Because if we can help save a child, then why couldn't that child save the family, the neighborhood, the community, the world.

POSITION SUMMARY
The Home-Based Case Manager is the first point of contact for all assigned families. This position is responsible for working collaboratively with the family, the home-based services team and the Child and Family Team (CFT) to develop and implement a treatment/service plan for all assigned families. The Home-Based Case Manager will coordinate services, assist with conflict management, provide parent education and life skills training, serve as an advocate for families, assist with emergency/crisis situations, and provide general case management over all assigned cases. The Home-Based Case Manager will be responsible for consistent communication with the family and placing agent, and will participate as a member of the CFT.

*Caseload will be no more than 12

PRIMARY JOB EXPECTATIONS

1. Case Management
• Makes initial contact with the family and conducts a face-to-face intake meeting within 48 hours of the referral.
• Coordinates the Family Assessment with the assigned therapist, as part of the intake meeting.
• Reviews the Family Assessment and works with the home-based services team and CFT to develop a treatment/service plan and safety plan to address the immediate needs of the family.
• Works with the home-based services team to coordinate all services needed for assigned families. Makes referrals to other community service providers, as needed, and ensures appropriate follow up and ongoing communication.
• Participates in case staffings with the home-based services team to monitor progress on goals.
• Helps to address questions, concerns and needed resources for the family. Provides advocacy services, as appropriate.
• Ensures that a member of the home-based services team makes face-to-face contact with every member of the family weekly.
• Assists with transportation services, as needed, to help the family meet identified goals.
• Participates as a member of the CFT and maintains regular communication with all parties.
• Participates in monthly team meetings with the home-based services team.
• Provides professional representation and feedback at all scheduled court hearings.

2. Skills Development
• Works collaboratively with the Homemaker/Parent Aid to provide skills training within the home. This could include goal setting, developing structure/time management, budgeting/money management, meal planning, behavior modification, parent education, etc.
• Observes and provides feedback as the family practices the skills learned
• Completes the Home Safety Checklist at each home visit. Ensures this checklist is completed by a member of the home-based services team weekly.
• Participates in an on call rotation to assist with crisis intervention services, as needed.
• Provides parent education services
• Provides supervised visitation, as needed.

3. Administrative & Supervisory Duties
• Sets up the initial case file and ensures all required documentation is completed timely, signed and on file. Works with the clerical assistant to start and maintain the Electronic Health Record (EHR).
• Provides training, supervision and general oversight of the Homemaker/Parent Aid(s).
• Creates a weekly schedule for the Homemaker/Parent Aid(s) and monitors levels of service.
• Completes a case note for all meetings, home visits, skills training sessions, CFT meetings, court hearings, phone contacts, etc. and enters this in the family's EHR.
• Assists with funding requests for concrete assistance, as needed.
• Maintains updated contact information for all assigned families, service providers, and placing agents.
• Helps to research community resources, build relationships with community agencies, and connect families to these resources within their home community.
• Completes Monthly Progress Reports for all assigned clients.
• Documents and reports any concerns or safety issues to the Program Director immediately.
• Assists with surveys and data collection.
• Reports any indications of child abuse to the Quality Assurance team and the Child Abuse Hotline.

EDUCATION & EXPERIENCE NEEDED
Must have a Bachelor's degree in social work, psychology, counselling or related area of study. Experience in home based services preferred.

Must be able to demonstrate awareness of the cultural and socioeconomic differences of our clients as well as value a nurturing family as the ideal environment; a commitment to empowering others to solve their own problems and a conviction about the capacity of people to grow and change; the ability to establish a respectful relationship with persons served to help them gain skills and confidence by maintaining a helping role and appropriately intervening to meet service goals; the ability to set appropriate limits.

 Ability to professionally represent YOC to community agencies
 Provides outstanding customer service and resolves issues/concerns in a timely manner
 Ability to prioritize and plan work to maximize efficiency
 Creativity in approaching situations

SUPERVISORY RESPONSIBILITIES
The Home-Based Case Manager supervises the Homemaker/Parent Aid

WORKING CONDITIONS
Typical work week is Monday-Friday 40 hours. It is expected that the Home-Based Case Manager will regularly travel to the clients' homes, attend CFT meetings, attend court hearings, etc. therefore working hours can be flexible. The Home-Based Case Manager will have regular contact with youth/families, placing agents, community agencies, etc. by phone, email, and in person. Courtesy and professionalism are expected at all times.

COMPENSATION
This position is full-time, and compensation includes a competitive salary and benefits plan.
