The QIDP is responsible for the development, writing, implementation, and follow-through with the Individual Habilitation Plan for each resident assigned to him or her. Plan individual services to provide for community based or community referenced training/leisure activities. Responsible for completion of all functions required in the operation of ICF/DD-16 bed facility. Responsible for supervising active treatment at all times.
A. Administrative - Establish a safe living and work environment. Ensure compliance to all OSHA regulations. Accountable for maintaining good sanitary condition in the Group Home assigned.
- Assure compliance with Federal and State Regulations for ICF/DD - 16 bed facilities.
- Maintain Quality Services and participate in on-going quality improvements programs for all individuals living in the Group Homes.
- Ensure all supplies are ordered for Group Home (i.e. Dietary, Cleaning, personal hygiene, active treatment, etc.) to provide for a well-run Group Home.
- Provide a schedule for Team Leaders to ensure adequate supervision of active treatment, safety of staff and clients, and program management. Keep an attendance record for all employees in the Group Home.
- Maintain and ensure accuracy of census reports. To be turned in on the first day of every month.
- Responsible to oversee all client personal funds and petty cash fund within the Group Home. Will ensure activity money/receipts are turned into the Business Office as per guidelines.
- Monitor and remain within established budget for Group Home.
B. Personnel - Participates in hiring, responsible for termination and evaluation of Team Leader positions and directly responsible for all activities carried out within Group Home assigned.
- Maintains and supports personnel policies approved and in practice by Beverly Farm Foundation.
- Responsible for scheduling training and implementation of inservice programs as needed in the Group Home.
- Review and ensure the accuracy of the weekly time sheet for employees in the Group Home. Turn in time sheet to the Business Office per established policy.
- Approve vacation, holidays, personal days, and early work releases for employees assigned. Arrange for coverage when Team Leaders call off or do not report to work.
- Conduct assessments of staff training needs. Implement staff training or appropriate support strategies to remediate, inadequate performance. Engage in activities consistent with increasing staff training and morale.
- Responsible to ensure Group Home personnel follow established policy and procedures and provide disciplinary action per the BFF code of conduct.
- Ensure staff receives training on all agency health and safety policies and procedures. Complete inservice sheet as needed according to yearly inservice schedule.
C. Active Treatment and Personal Care - Be an advocate for the resident and report any incidents of abuse/neglect or rights violations to the appropriate party.
- Prepare necessary assessments prior to Annual IHP Meeting (i.e. SIB, CI level, Recreation/Leisure, Self-Medication).
- Responsible for the coordinating the efforts of the Interdisciplinary Team to ensure that each client's program plan is geared to his/her individual needs. Chair the Interdisciplinary Team meetings, write the individual plan, and implement the program plan within 14 days of holding the IDT meeting to ensure accordance with the policies and procedures. Train all disciplines on each individuals program plan upon completion. Coordinate activities between Group Home, Main Campus, and Developmental Training programs to ensure universalizastion. Coordinate adequate transportation to meet client's needs as specified by the IHP.
- Train all staff in providing functional, outcome based habilitative activities and ensure training opportunities. Provide performance feedback to assigned staff regarding the quality of active treatment and programming being provided.
- Responsible for records for the individuals served in the Group Home, the timeliness, accuracy, completeness, of IHP's, progress notes, monthly Q Interdisciplinary Note reviews and other documentation related to the Group Home required by regulations.
- Coordinate the services needed by each individual with the facilities consultants. Ensure these services are provided and necessary follow-up is completed.
- Oversee the dietary service in the Group Home.
- Supervise the day to day operations of the Group Home.
- Complete the 6-month review by the end of the designated month for all clients in the Group Home.
- Ensure all medical needs of clients in Group Home are being followed.
- Ensure accuracy and completion of the Group Home documentation system.
- Ensure building maintenance is completed, coordinate with Group Homes Maintenance personnel.
- Responsible for scheduling and completing monthly activities calendar and will coordinate community activities along with activities in home. Will ensure residents go out into the community at least 2 times per month.
- Establish and maintain home like atmosphere.
- Conduct routine observations of the DT program and address concerns with the program coordinator and document in the client records.

E. Committee/Meeting Responsibilities - Will actively participate in the Human Rights/Behavior Management Committee for the Group Homes and complete necessary documentation.
- Will participate in the Infection Control/Pharmacy Committee for the Group Homes and complete necessary documentation.
- Attend and participate in Group Home Administrative Meetings.
- Conduct and keep minutes of regularly scheduled shift meetings for Group Home personnel.
F. Other Duties as Assigned Qualifications Must have a 4 year degree related to Human Service and 1 year experience working with ID/DD individuals. Previous supervisory training or experience preferred.
- Willingness to accept supervision and directions and follow specific programming techniques.
- Ability to effectively communicate verbally and non-verbally.
- Ability to be a contributing team member and work cooperatively with staff and agency personnel, families, and community.
- Sufficient physical stamina and mobility for lifting, carrying, pushing, and pulling.
- Ability to stay calm and not become angry when working with individuals served who are upset.
- Ability to supervise and train individuals with developmental disabilities in a mature and responsible manner.
- Ability to supervise and train Group Home personnel in a mature and responsible manner.
- Ability to read, write, and perform basic math skills.
- Prefer License or proof of eligibility for insurance by possessing a good driving record.
