Candidates must be located in and have an active/valid license to practice Pharmacy in the state of Pennsylvania.

This role offers the opportunity to make a meaningful impact in the lives of hospice patients by supporting palliative care initiatives, ensuring safe and effective medication use, and improving overall quality of life at end-of-life. Consultative Pharmacists work collaboratively with nurses and caregivers to provide real-time, patient-centered consultation and support.

Essential Functions

  1. Provide expert guidance on pain and symptom management tailored to individual patient needs and actively contribute to enhancing patient comfort and dignity during end-of-life care.
  2. Collaborate directly with nurses and hospice staff to develop effective, cost-conscious medication care plans.
  3. Educate healthcare professionals and caregivers on medication usage and adverse event prevention.
  4. Accurately document clinical interventions and patient outcomes in electronic health systems. Ensure all encounter documentation is complete, clear, and ready for transcription/processing.
  5. Maintain and update patient medication profiles and records in accordance with HIPAA and regulatory requirements.
  6. Consistently meet or exceed expected productivity benchmarks.
  7. Demonstrate exceptional communication and interpersonal skills in a remote or hybrid work environment.
  8. Maintain a professional, empathetic tone in all patient and team interactions.
  9. Engage with a multidisciplinary care team in a fast-paced call center environment.
  10. Support team success through shared learning, collaboration, and knowledge sharing.
  11. Participate in weekend rotations and contribute to team coverage as needed.

  2. Performs other related duties as assigned or apparent.

Supervisory and Managerial Responsibility

  • This position does not have supervisory or managerial responsibilities.

Knowledge, Skills & Abilities

Education, Licensure or Certification:

  • PharmD or B.S. in Pharmacy from an ACPE-accredited school of pharmacy
  • Active pharmacy license in home state (Pennsylvania or Tennessee)
  • Pharmacy licenses in other states or willingness to obtain additional licenses

Work Experience or Related Experience:

  • Experience in hospice, palliative care, or oncology settings preferred
  • Previous experience working in a call center or remote clinical consultation environment preferred
  • Familiarity with pharmacy benefit management (PBM) services preferred

Specialized Knowledge, Skills & Abilities:

  • Demonstrates competency in equianalgesic dosing, generic and therapeutic substitution, formulary management and prior authorization procedures
  • Demonstrates understanding, competency and adherence to all state and federal pharmacy practice rules and regulations
  • Strong clinical assessment and decision-making skills
  • Excellent written and verbal communication skills
  • Proficient typing and computer skills are paramount to employee satisfaction and success while fulfilling responsibilities
  • Excellent listening skills
  • Proven ability to manage multiple projects simultaneously while paying strict attention to detail

Equipment:

  • Working knowledge of a PC, business and communications software (MS Office) and web-based tools is required

Work Environment, Conditions, and Demands

  • Work is performed in a remote or hybrid office environment.
