Job Description

  1. COMMUNITY EDUCATION AND HEALTHCARE ACCESS: Collaborates with a variety of community members and stakeholders, private and public organizations, and government agencies to improve the health of communities. Facilitates evidence-based programs (diabetes prevention, fall prevention, chronic disease self-management etc.) to community members to promote positive lifestyle behaviors and safety.
  2. COMMUNITY SCREENINGS AND IMMUNIZATIONS: Attends screening and immunization programs (which may take place on evenings and weekends) with community outreach manager, community health nurse coordinator, medical center providers and team members, volunteers and community partners. Administers community-based vaccinations, such as flu shots and COVID vaccines. Documents appropriately, keeps vital records, adheres to infection control standards and maintains a safe, clean, and orderly environment. Responds effectively in an emergency situation by initiating emergency measures, including CPR and calling 911.
  3. OUTREACH RESPONSIBILITIES: Attends community events, which may take place on evenings and weekends, to educate the community on health-related topics. In conjunction with other department team members, serves as a community health representative for community-based organizations and agencies.

 

Community Health Per Diem Pool Guidelines

 

  1. Schedules are considered a commitment. Any event, cancelled, without seven (7) days’ notice, is considered an occurrence. Two occurrences in six (6) months may result in removal from the Community Health Per Diem pool.

 

  1. Must work a minimum of two (2) events per month and a minimum of two (2) cholesterol events per year. Depending upon the number of events and size of the pool, additional shifts may or may not be available. 

 

  1. One of the shifts required must be a weekend event per quarter: four (4) events per year.

 

  1. All Community Health Per Diem Pool staff must have current CPR certification.

 

  1. To the extent applicable, all Community Health Per Diem Pool staff must have/maintain current Maryland licenses and certifications. 

 

 

Qualifications

  • Licensure as a Registered Nurse in the state of Maryland, or eligible to practice due to Compact state agreements outlined through the MD Board of Nursing, is required.
  • 1-2 years of relevant experience in a Community Health related field (preferred)
  • Required CPR Certification
  • Required State Driver’s License
