Department: Clinical Performance and Quality Improvement (CPQI)
Hours per week: 40
F/T or P/T status: Full-time
Work schedule/hours: Monday - Friday, 8 am - 5 pm
Job Summary:
Proactively works to improve the health status of identified populations with chronic conditions. Implements, promotes and participates in wellness and health related activities. Implements evidence based interventions in consultation with the Director to improve health outcomes of patients with chronic disease and to meet national performance standards.
Minimum Qualifications:
Florida RN Nursing license and CPR certificate required. Must occasionally lift and/or move up to 200 pounds (pushing a patient in a wheelchair).
A minimum of five years of related experience is required.
