The Clinical Consultant for HCM, Case Management Operations focused on the transition to the Carport system implementation
This role is a clinical subject matter expert dedicated to ensuring the successful deployment and optimization of case management technology
This role is primarily responsible for providing clinical system guidance to successfully execute on Case Management workflow efficiencies and optimization of the new technology
The role will identify current gaps and pain points ensuring the build and implementation of the case management technology will provide efficiencies for the users
The role will also capture and document business requirements from case management leadership to ensure the system build meets current and future needs.
The Clinical Consultant is a member of implementation team who will work directly with the Hospital Case Management Director and Program Director, CMBT to ensure seamless system design, development, and implementation.
The Carport Clinical Consultant must possess excellent communication skills, proven motivational capabilities, be a forward thinker, and have an executive presence
This individual can work effectively with senior organizational leaders across a diversity of work styles and personalities.
Major Responsibilities:
Provides ongoing assessment of system standards, processes, and practices to ensure Hospital Case Management technology supports best practice and remain adherent to current industry standards and regulatory requirements.
Serve as a clinical subject matter expert, guiding the implementation team in designing and developing system standards that align with case management needs.
Participate in department – wide work teams to identify best practices and determine options and opportunities for optimization, functionally and workflow.
Collaborate closely with the project leadership, particularly with Hospital Case Management Director and Program Director, CMBT, to ensure seamless system design, development, and implementation.
Identify workflow inefficiencies and recommend solutions to optimize the case management process.
Capture and document business requirements from case management leadership to ensure the system build meets current and future needs.
Partner and assist CMBT, ITG, Education, and other key stakeholders.
Assist project management to ensure all considerations are built into enterprise wide project plans.
Escalate any identified barriers / risks that could impact the project.
Hold strong organizational skills and the ability to proactively prioritize needs to effectively manage resources.
Demonstrate the ability to foster team building and a collaborative environment.
Supports Hospital Case Management with future technology automation partnerships and processes.
Education & Experience:
Bachelor’s degree in Nursing (RN)
Required
Master’s degree in Nursing (RN)
Preferred
3+ years of experience with system application
Required
3+ years of experience in Hospital Case Management
Required
Experience with Carport
Required
Experience with electronic medical record (Me ditech, Epic, Cerner)
Required
Licenses, Certifications, & Training:
Registered Nurse - Currently licensed as a registered professional nurse in the state in which he or she practices, in accordance with law and regulation.

Advanced computer skills and proficiency in Microsoft Office suite of product
