The Hospice RN Case Manager position in MN offers part-time or full-time roles with a pay starting at $42+ DOQ.

• Responsibilities include coordinating hospice care, evaluating service effectiveness, consulting with physicians, and ensuring care plans are current.
• Perform comprehensive patient assessments, supervise staff, and participate in team meetings and on-call duties.
• Must have a nursing degree, current RN license, and experience in long-term care, homecare, or hospice preferred.
• Require excellent communication skills, good decision-making, basic computer skills, and a valid driver’s license with insured vehicle.
