Retention Incentive: As a newly hired Physician .50FTE. You will receive: $10,000 retention incentive after (1) one year of continuous regular employment. $10,000 retention incentive after (2) two years of continuous regular employment. $10,000 retention incentive after (3) two years of continuous regular employment. All retention incentive bonuses are based on FTE. The retention incentive is taxable, and all regular payroll taxes will be withheld. At Corrections Health, we provide medically needed health care to adults and youth in custody at Multnomah County Jails and Juvenile Detention Facility. The Addiction Medicine Physician will act as an in-house clinical expert in the Corrections Health Division in Addiction Medicine, principally tasked with oversight of policy and protocol for Medication Supported Recovery (MSR) in Corrections Health. The Addiction Medicine Physician will consult with staff on complex substance use cases, lead a more standardized approach to treatment in Corrections Health, support alignment with community standards, evaluate and plan for expansion of eligibility for adults in custody. This position will oversee new medication options including methadone, extended-release buprenorphine injectables, and treatments for stimulant use disorder, and oversee expansion to additional sites including the Department of Community Justice and Harm Reduction Clinic. This position may support expansion through video medical consultation to the nurses, nurse practitioners and physician assistants. Other duties include but are not limited to the following: Clinical Care Diagnose and treat acute and chronic medical conditions according to scope of practice to include: Conducting a medical history, Conducting physical exams Ordering and interpreting diagnostic tests, Devising a course of treatment or clients and involves other resources as necessary for quality care Discussing alternative treatment, risk and benefits of treatment, Prescribing and monitoring medications, Appropriate counseling of patients and reassessment of the patient at appropriate intervals. Utilizes panel based information technology tools to prioritize and direct proactive care for patients Preventive health screening and chronic disease care management. Follow-up on utilization issues (ED, hospital admissions). Respond to in jail emergency situations in the clinic or housing and develop plans of care. Provide holistic care to people who use drugs that may include medications such as HCV treatment, contraception, HIV Pre-Exposure Prophylaxis, doxy-PEP, and antibiotics Consultation/Coordination Provide clinical consultation to other medical providers, Nurse Practitioners, Physician Assistants, Community Health Nurses and others as needed. Remain available to answer questions and provide direction in difficult or unusual situations and provide clinical consultation for new staff or those who have questions or need additional support Prioritize and direct activities of the care team. Coordinate with Behavioral Health and Transition Services Program providers to effectively integrate care and assure continuity for patients exiting the facility Stakeholder engagement and relationship development with Multnomah County Sheriff’s Office to increase confidence and partnership with the Corrections Health MSR program. Give input into procedures and protocols for management of common substance use disorders and corresponding withdrawal syndromes and developing training tools for clinical staff. Provides coverage for clinical colleagues in their absence. Attend provider meetings as scheduled. Attend shift reports and contribute to patient care planning and identification of patient needs. On call for provider coverage 24 hours/day as scheduled on the monthly calendar coordinated by the Medical Director. Program Evaluation and Structure Research and evaluate current addiction medication practice in Corrections Health and other agencies for best practices and legal requirements. Evaluate and assess current medication supported recovery policies, practices, and procedures for improvements, provide clear directives to staff for medication levels, best medical treatment, or actions, staffing levels for medication administration, etc. Direct staff and teams on policy changes and best/promising practices. Collaborate with staff and team members to gather, track and evaluate data of medication use, dosage, adverse reactions, observed behavior changes, etc. per location. Partner with department and evaluation staff to identify priority MSR outcomes and workflows for evaluation and improvement. Contribute to iterative improvement of overdose response and partner with other health department stakeholders to provide technical assistance on overdose response for Corrections sites. Contribute to clinical workflows and protocols for Advanced Practice Clinician MSR continuation and initiation at the community Harm Reduction Clinic Create and maintain youth/adolescent MSR protocols for the Juvenile Detention Center Set performance standards and scope of practice requirements per job profile/license of position to guide clear directions and supervision requirements of each medicine event. Evaluate staffing and scheduling patterns and needs per location and shift and provide communication to leadership on staffing successes and challenges. Minimum Qualifications/Transferable Skills*: Education/Training/Experience: Graduation from an accredited school of Medicine AND One (1) year residency in a primary care setting 2+ years experience in Addiction Medicine Certification: Possession of, or ability to obtain, an appropriate and valid license to practice medicine in the State of Oregon and ability to obtain credentialing approval.
